This survey is part of the Waikato District Health Board Community Health Forum Evaluation. It aims to explore if the Community Health Forum mechanism keeps communities involved and informed of Waikato District Health Board activities and issues; and to explore if the current Community Health Forum mechanism is reaching the intended target group(s). 

The evaluation is being conducted by researchers from the School of Psychology at the University of Waikato. It has been approved by the School of Psychology and Ethics Committee of the Faculty of Arts and Social Sciences, University of Waikato.  Any questions about the ethical conduct of this research may be sent to the convenor of the Research and Ethics Committee (see contact details below). 

This survey should take approximately 5 minutes to complete. 

Eligibility for this survey includes ALL Waikato district residents only. 

If you are a Waikato district resident, upon completion of this survey, you will be eligible to enter the draw to win 1 x $100 supermarket voucher. 

All information you provide in this survey is anonymous. You will in no way be identifiable. Please note that your completion of this survey will be regarded as informed consent. Once you have completed this survey, you will not be able to withdraw any of the information you provided as you are in no way identifiable to us.
